Admissions and Funding

Individuals may be referred by behavioral health, rehabilitation, social service,  educational service providers  or insurance companies. Self referrals are also accepted as long as a funding source can be identified. Northern Transitions, Inc. is an accepted Employment Network, registered with Social Security to provide Ticket to Work services. Admission is first-come first-served based on date of receipt of all application data.  Reasonable accommodations will be made for any individual meeting entrance criteria who wishes to participate.

It is the policy of Northern Transitions, Inc. to provide accessible services and to continuously develop policies and practices that promote accessibility.

School to Work Transition and Work First Services have special admissions procedures.
